PCMH Kids

BlueCross BlueShield of South Carolina is pleased to announce the update and expansion of our pediatric patient-care medical home (PCMH) program, PCMH Kids. The new program will launch Jan. 1, 2021 and the provider network will be open to all eligible pediatric primary care practices in South Carolina. Participating providers will receive monthly care management fees (PMPMs) and opportunities to earn financial incentives based on quality performance.
